MEMORY CARE

Caring for the needs of those with memory loss provides a remarkable opportunity for ministry and has become one of Angels Care Center’s most highly respected programs.

Specially trained staff interact with residents through various stages of the disease to help them function at their highest levels. Encouraging life skills and daily activities promotes a sense of purpose and reassures families that their loved ones are cared for in a warm, tender, supportive environment.

Angels Care Center supports and stays connected to the Alzheimer’s Association, receiving continuous updates and training on new studies and new methods of treatment.

TRANSITIONAL CARE

Angels Care Center is located directly across the street from the Cannon Falls Medical Center, Mayo Health System.  This location makes Angels an excellent choice for individuals rehabilitating from illness or injury.

A customary stay in transitional care ranges from one week to several weeks and is focused on providing a therapeutic and continuous progression toward recovery from hospital to home.  To achieve that goal, a transitional stay may include:

Professional Services

Physician-led rehabilitation team:

  • Nursing
  • Physical, Occupational and Speech Therapy
  • Social Services
  • Therapeutic Recreation
  • Nutritional Services

Discharge Planning

  • Individualized chronic disease management education
  • Therapy staff can continue therapy after discharge in the patient’s own home through cooperative agreements with Cannon Falls Home Health.

Accommodations

  • Spacious, attractive and comfortable rooms
  • Flat screen TV and telephone for each resident staying in the transitional care unit
  • Attractive dining room and visiting lounges
  • User-friendly, ability-adaptive computer with internet and email access

CONTINUING CARE

Since 1977, Angels Care Center has served Cannon Falls and the surrounding communities by providing services and continuing care for those in need.

Conveniently located at the intersection of State Highway 52 and Highway 19, Angels Care Center is a residence that offers a homelike environment and kind, experienced staff.

The Chandelier lounge offers an inviting and comfortable place for visiting, reading or simply relaxing.  The peace Chapel, opened in 2007, is a beautiful place of repose where residents and families can find refuge and solace; a place where church services are conducted without distraction.  Visitors are welcome to use the adjoining Family Room to visit with a loved one, share a meal or even cook one together using the small kitchenette.  The Family Room is an inviting place for birthday parties, small group activities, social gatherings or casual lounging.

Creating a family atmosphere is our key to making everyone feel welcome, comfortable and cherished.  Meals are provided in the Garden Trellis or Oasis dining rooms.  Daily activities are offered for individual or group enjoyment.  Our large patio offers a waterfall pond, raised vegetable and flower gardens and comfortable patio furniture for outdoor enjoyment.

Residents and their families can have peace of mind trusting their loved one’s need for care and support to competent, compassionate staff around the clock.  Special care planning is designed to support and enhance end of life experiences.
